Loan deferments, health insurance forms, make-up exam forms, readmission forms, Pass/Fail applications, application for degree, transcript requests, graduate specialization forms, change of major forms, change of address or ss# forms, undergraduate course substitution forms, high school exemption request forms, withdrawal forms, and minor forms should be submitted to the One Stop Center located
on the 1st floor of North Hall.
If necessary, an appointment can be made with a Registrar's Office Staff at the One Stop Center.
